340B PROGRAM: The Glue That Should Hold Our Healthcare System Together (Introduction)

The 340B Drug Pricing Program, a federal program designed to help poor patients access affordable healthcare, should literally be the glue holding the American healthcare system together.

The program touches more than 42,000 healthcare providers—ranging from hospitals and rural health centers, to hemophilia and family planning clinics—and more than 32,000 pharmacies. In 2025, healthcare providers purchased over $100 billion in outpatient prescription drugs, largely fueled by hospitals. Yet, despite this massive growth, patients continued to be squeezed with medical debt, running contrary to the program’s promise.

Our healthcare system’s high interdependence means that a weakness in one sector can have widespread repercussions, so the 340B Program is incredibly important. Rising waste, fraud, and abuse are posing an increasing risk.

For that reason, a healthcare program of this size and magnitude cannot be entrusted to good faith alone; rather, improved transparency guardrails are needed to ensure its success and ultimately its intended beneficiaries: patients.
